WHAT IS NURSING HOME HEMODIALYSIS?

Hemodialysis services that are provided to patients with End-Stage Renal Disease (ESRD) - and other kidney disorders where dialysis is the prescribed treatment - in the comfort of their long-term skilled nursing facility.

DO PATIENTS/OUR RESIDENTS GET ANY COVERAGE FOR TREATMENT EXPENSES VIA MEDICARE/INSURANCE?

Home Dialysis is paid for by most major insurances, Medicaid and Medicare Part B.

ARE THERE ANY OTHER FEES ASSOCIATED WITH RUNNING A DIALYSIS PROGRAM?

Concerto is required to charge a small staffing supplement. This fee varies depending on the vision you have for your program and the level of care required.

WHAT KIND OF HEMODIALYSIS SERVICES DO YOU PROVIDE?

WHAT LEVELS OF ACUITIES DO YOU ALLOW IN DIALYSIS?

Not withstanding State-specific regulation, Concerto services patients with acute/clinically complex situations. We currently operate many vent dialysis units with the capacity to treat complicated patients. We have worked with many patients confined to long term acute care hospitals when beginning our treatment, who now are living comfortably in a nursing home receiving dialysis treatments.

CAN A PATIENT MEET A SOCIAL WORKER FROM THE CONCERTO TEAM APART FROM THE TREATMENT SESSIONS?

Yes. Our dedicated social workers can meet you before or after your treatment session privately.

Since we are exclusively working with skilled nursing faculties, we do not a pre-set list of conditions for potential clients. The terms of an agreement are thoroughly discussed before a contract is executed.
